Vaccine Schedule
The following is our office's routine, recommended vaccine schedule:

  • First visit: Hepatitis B vaccine if not given at hospital

  • 1 month: No vaccines due

  • 2 months: DTaP-IPV-HepB, Hib, PCV13, RV5

  • 4 months: DTaP-IPV-HepB, Hib, PCV13, RV5

  • 6 months: DTaP-IPV-HepB, Hib, PCV13, RV5

  • 9 months: No vaccines due

  • 12 months:  MMR, Var, PCV13, Hepatitis A (also anemia screen, lead test

  • 15 months: DTaP, Hib

  • 18 months: Hepatitis A #2

  • 2 years: anemia screen, lead test

  • 4 years: MMR-Var, DTaP-IPV 

  • 11 years: Tdap, MCV4 and HPV4

  • 16 years: MCV4 #2

Vaccine Abbreviations

 

  • DTaP: Diphtheria-tetanus-pertussis

  • IPV: Inactivated polio vaccine

  • Hib: Haemophilus influenzae B

  • PCV13: Pneumococcal (13 strain)

  • RV5: Rotavirus (5 strain)

  • MMR: Measles-mumps-rubella

  • Var: Varicella vaccine

  • Tdap: Tetanus-diphtheria-pertussis

  •  (teenage-adult version)

  • MCV4: Meningococcal (4 strain)

  • HPV4: Human papilloma virus (4 strain)
